Senior Adult Independent Living (SAIL) Case Management Program
Senior Adult Independent Living (SAIL) helps individuals in the Phoenix area live independently in their own homes by coordinating needed services and assistance with daily living activities. The program, sponsored by the Area Agency on Aging, Region One, a private non-profit corporation, provides case management services to individuals age 60 and older and individuals aged 18-59 with a diagnosed disability. In home services available include: home delivered meals, personal care, homemaking services and home nursing.
The purpose of Senior Adult Independent Living is to provide professional, compassionate care to individuals while maintaining independent living.
SAIL serves older and disabled adults, families, and caregivers.
Primary services include case management, adult health care, home delivered, home nursing, personal care, and homemaking.
